Community Counseling & Mediation’s OASAS Outpatient Treatment Program ASAP (Addiction and Substance Abuse Program) now operates out of CCM’s Elm Place Clinic.
Focused Service Delivery:
The clinic plays a vital role in addressing the needs of our supportive housing clients and community members who are actively developing skills to manage triggers and cravings related to substance use. We are dedicated to providing timely access to critical services for clients across our four supportive housing facilities.
Comprehensive Program Offerings:
Our program offers substance use counseling and Medication-Assisted Treatment (MAT) to support recovery and overall well-being. These services are available not only to clients in our supportive housing programs, but also to individuals from the broader community. We are committed to providing coordinated, client-centered care to all who seek support.
Program Focus Includes:
- Identifying personal triggers and emotional reactions
- Implementing healthy coping mechanisms and techniques
- Practicing alternative responses to aggression and conflict
- Crafting personalized anger management plans that suit individual needs

Our Services
- Medication-Assisted Treatment (MAT): Guided by our team of highly trained doctors and counselors, we provide FDA-approved medications in conjunction with comprehensive therapy, forming a robust framework for sustained recovery.
- Holistic Psychiatric Medication & Counseling: Additional resources to address and support various mental health challenges
- Buprenorphine (Suboxone): Designed to alleviate withdrawal symptoms and cravings
- Naltrexone (Vivitrol): A monthly injection aimed at preventing relapse and supporting long-term sobriety
- Anger Management Program CCM’s comprehensive 12-week, courtapproved Anger Management Program is designed to equip individuals with effective strategies for navigating anger and stress.
