Community Counseling and Mediation’s Minority Outreach Community Health Access (MOCHA) program is federally funded by the Substance Abuse and Mental Health Administration (SAMHSA) as part of the Minority AIDS Initiative - Continuum of Care grant. 

MOCHA is a case management outreach wellness program that targets minority populations. We support individuals with health problems in need of comprehensive case management services in Central Brooklyn areas. Our federally funded program provides our 278 unduplicated clients with a co-location of behavioral health and medical services. We serve 278 unduplicated minority men and women annually who receive holistic, culturally and linguistically sensitive counseling services that utilize evidence-based theories of Motivational Interviewing, Cognitive Behavioral Therapy, and Personalized Cognitive Counseling. In partnership with SUNY Downstate’s HEAT Program, our participants receive medical services on-site. 

The primary goal of MOCHA is to serve individuals who may lack the resources and support they need to be fully functioning and healthy members of society. Our aim is to provide an array of different services to best support our clients to reach physical and psychological health, stability and well- being. 

The program provides case management services to support clients who may need assistance with referrals, benefits applications, health checks, outpatient treatment, and culturally sensitive individual, group and family counseling.

MOCHA provides individuals 18 and older with assistance in the following areas:
